I certify that all information I have provided in order to apply for and secure work with Four Seasons Roofing is true, complete, and correct.
All employees, contractors, and lessees are required to adhere to all Four Seasons Roofing policies, including the Drug-Free Workplace Policy. I understand that Four Seasons Roofing maintains a drug- and alcohol-free workplace and requires a post-offer/pre-employment drug screening and physical exam as a condition of employment. I acknowledge that failure to complete the pre-employment testing, a positive test result, refusal to test, or submission of an adulterated or substituted specimen will result in disqualification from employment consideration.
I authorize the laboratories, facilities, physicians, nurses, and technicians contracted by Four Seasons Roofing to collect necessary specimens for the purpose of determining the presence or absence of controlled substances. Additionally, I authorize the disclosure of testing results only to the appropriate company officials, understanding that Four Seasons Roofing is responsible for maintaining the confidentiality of all testing results. In connection with my application for employment (including contract for services or volunteer services), consumer reports may be requested. These reports may include information regarding previous employers, salary, work experience, education, accidents, licensure, credit (where permitted), and more. Public record information such as driving records, workers’ compensation claims, judgments, bankruptcy proceedings, and criminal records from federal, state, and other agencies may also be included. Additionally, investigative consumer reports, as defined by the federal Fair Credit Reporting Act, may be gathered through personal interviews with former employers and associates to obtain information about my work performance, character, general reputation, and personal characteristics.
I expressly authorize, without reservation, Four Seasons Roofing, its representatives, employees, or agents to contact and obtain information from all references (personal and professional), employers, public agencies, licensing authorities, and educational institutions to verify the accuracy of all information provided in my application, resume, or job interview. I hereby waive any and all rights and claims I may have against Four Seasons Roofing, its agents, employees, or representatives for seeking, gathering, and using truthful and lawful information during the employment process, as well as against any individuals, corporations, or organizations furnishing such information about me.
I understand that Four Seasons Roofing does not unlawfully discriminate in employment and that no question on this application is used to limit or exclude any applicant from consideration for employment on any basis prohibited by local, state, or federal law.
I acknowledge that Four Seasons Roofing follows an at-will employment policy, meaning that either the employee or the employer may terminate the employment relationship at any time, for any reason, with or without notice. I also understand that this application, along with any materials discussed during an interview, does not create an employment contract, expressed or implied. Furthermore, I agree that any future changes to duties, compensation, working conditions, benefits, or company policies and procedures will not alter this at-will arrangement.
For positions requiring a CDL or Non-CDL driver’s license, I understand that I must maintain a current and valid driver’s license at all times. I also acknowledge that any omission, misstatement, or false information on this application or any document used to secure employment with Four Seasons Roofing will be grounds for rejection of my application or immediate dismissal if employed, regardless of when the information is discovered.
I understand that this application remains active for 60 days. If I have not been contacted by Four Seasons Roofing within that time and still wish to be considered for employment, I will need to reapply by completing a new application.
Additionally, I understand that if hired, I must provide proof of identity and legal authorization to work in the United States as required by federal immigration laws and must complete an I-9 Form for this purpose.
